Superseded statements are kept and marked, not deleted. That is what lets a later answer be identified as quoting last year's rule rather than simply being “wrong” with no explanation of how it got there.

- When a rule started applying. Without this, a correct answer and an outdated one look identical.
- Jurisdictions
- Where the statement holds. Most damaging service errors are a true statement applied to the wrong place.
- Superseded-information handling
- The prior version is retained and marked superseded rather than deleted — so an answer citing it can be identified as citing the old rule.
- Responsible owners
- Who owns each statement, so a flagged error routes to the team that can actually correct it.
Flagged against the approved record

Incorrectness
The answer states something your approved record contradicts outright.
Omissions
The answer is not wrong, but leaves out a condition that changes the outcome for the reader.
Outdated rules
The answer reflects a statement that has an effective date and has since been superseded.
Contradictions
Two answers — or two parts of one answer — cannot both be true given the approved record.
Missing conditions
An entitlement or requirement is stated without the qualification that scopes it.

Publishing a correction is not proof an AI system used it.
